Probably Assassin's Creed Odyssey. It's got everything you mentioned and if you play as Kassandra one of my favourite protagonists.
I’m trying to decide between Dragon’s Dogma 2, Enshrouded, Oblivion Remastered, and Elden Ring. I’ve already played The Witcher 3 and Skyrim, and what I enjoy most is a beautiful open world with rewarding exploration, lots of loot, a wide variety of weapons and armor, and plenty of character builds to experiment with. I love discovering hidden caves, ruins, dungeons, unique gear, and powerful weapons while exploring, similar to the experience I had in The Witcher 3 and Skyrim. I also prefer real time action combat and don’t enjoy turn based games. Based on those preferences, which game would you recommend and why? If you have any other suggestions that fit what I’m looking for, I’d love to hear them as well.

Definitely Elden Ring. Although if you've never played a fromsoft game before, it's gonna be challenging at first. To give some tips without spoiling anything, I'd say: -leveling vigor is the most important attribute BY FAR and it's not even close -level up your other attributes to meet weapon requirements, then focus on vigor -upgrade your weapon as often you can, this is the main way to increase your damage -take some time to understand the mechanics and the flow of combat. When it clicks, you'll have an easier time.
i enjoy challenging games, but i wasnt able to get past the challenge that is elden ring. i just couldn’t figure it out. i WANT to like the game, but man it is HARD
Horizon Zero Dawn maybe?
Skyrim. But hear me out Gate to Sovergard mod pack has 1900+ mods and completely changes the game. It has everything you’re asking and more.
Crimson Desert is for you!
He said beautiful game with good story and exploration, cd sucks innall of those parts
If you like souls games and don’t mind the challenge then go Elden ring. Tons of hidden weapons and hidden caves in that game and a beautiful world. If you don’t want the added challenge go dragons dogma it has tons of hidden caves and also a beautiful world but the loot often comes more from vendors but a lot of it can be found in hidden caves as well.
Out of those, Elden Ring is the clear winner imo, genuinely one of the best games ever made of any genre
Horizon: Zero Dawn, then the sequel Horizon:Forbidden West
Crimson Desert!!!
Elden Ring is by far my favorite from what you e listed
Elden ring is perfectly what you asked for. It’s also the best game you listed. That said, it’s far more difficult than the other games, you’re gonna have to be ok with dying. But if you push through, you’ll find a masterpiece.
Divinity original sin 2, kingdom come deliverance
i wanted to throw kingdom come deliverance 1&2 out there. 2 of the best games ive ever played. they are challenging at first, especially the first game. but you get much stronger as you grow from a peasant to a knight.
