mathematically, worm scarf is better. realistically, theyre identical except for very specific scenarios.

Honestly both are good, I think BoC is better late game but I dont think itd stay in my slots permanently bc I prefer other stuff
If you can stack dodge it's probably better then the scarf But the scarf is more reliable
The crowd control of the Brain is kind of nice when you're doing events or there's a lot of mobs.

Brain of Confusion with Hallowed Armour and Star Veil is about as close to invincibility as you can get in this game.
Scarf damage reduction is 17% , brain dodge is 1/6 chance (16.67%) so roughly the same. A single attack of 100 damage means that it's 83 with the scarf, and 100 or 0 with the brain. Versus single high damage, brain seems a bit better. With multiple attacks that advantage disappear But this 1/6 is guaranteed or not? I mean if I'm facing 6 attacks, the 5 first not dodged, the 6th is guaranteed to be dodged or not? If yes, it's the same as the scarf, if not it may be a bit worse
Also worth noting that when dodging you get to keep you health regen ramp up while worm scarf will always reset when hit
i prefer the consistency of the scarf, but the brain is better if you're addicted to gambling
Worm Scarf gets significantly more effective the more additive DR you have, and even at base it's already arguably better. The one thing the Brain Scarf used to truly have going for it was daytime EoL but even that was taken from it so I really think the Worm Scarf is just strictly better in the current game.
Depends on the situation, build, and playstyle. I lean more towards scarf in general, but I also only play on master mode. It's guaranteed defense.
